Updated 2025 Pharmacists' Patient Care Process PPCP

A Journey Through Clinical Pharmacy- From Books to AI

The field of clinical pharmacy has transformed dramatically over the past 15 years, driven by advancements in digital tools, mobile apps, and, most recently, artificial intelligence (AI). As someone who has witnessed these changes firsthand—from my days as a Master of Pharmacy (MPharm) student at The University of Manchester to my current role as a clinical pharmacist—I want to reflect on how our access to medical information has evolved and what it means for both pharmacists and the public.

 The Era of Textbooks and Online Databases

When I started my MPharm studies in 2010, the British National Formulary (BNF) and BNF for Children were my go-to references. These thick books contained essential drug information, and every pharmacy student had them on their desk. While they were invaluable, they had limitations: updates were only published biannually, and searching for specific information could be slow.

To supplement my learning, I frequently used MedicinesComplete, Google Scholar, and PubMed. These online platforms provided access to research papers and clinical guidelines, but finding the right information required patience and advanced search skills. At the time, this felt like cutting-edge technology—yet, looking back, the process was time-consuming compared to today's instant-access resources.

The Rise of Mobile Apps in Pharmacy Practice

As I transitioned into clinical practice in 2014, I quickly realized the need for faster, more accessible drug information. Mobile apps revolutionized how pharmacists worked. Instead of flipping through physical books or sitting at a computer, I could now access trusted drug databases at my fingertips.

I started using Medscape, Micromedex, Lexicomp, and UpToDate to look up drug interactions, treatment guidelines, and evidence-based recommendations. These apps significantly improved efficiency, especially in high-pressure environments like hospitals and clinics. Additionally, the BNF and MIMS apps provided updated drug information, making printed versions almost obsolete.

This shift allowed pharmacists to provide quicker and more accurate advice to both healthcare professionals and patients. The convenience of having critical information on a smartphone meant we could make informed decisions in real time.

 The AI Revolution in Pharmacy

Then came the AI wave in 2022. The introduction of tools like ChatGPT, Gemini, Copilot, Grok, Perplexity, and DeepSeek transformed how pharmacists access and process information. Unlike traditional apps, which provide static drug monographs, AI tools allow for dynamic conversations, summarization of research papers, and even predictive analysis of drug interactions.

For example, instead of manually searching PubMed for recent studies on a specific treatment, I can now ask an AI tool for a summary of the latest evidence. If a doctor asks for recommendations on an unusual case, AI can help analyze multiple guidelines in seconds, providing a well-rounded response.

However, AI tools are not perfect. They require careful validation and should always be used alongside trusted references. While they have significantly improved the speed of information retrieval, clinical judgment remains essential. Pharmacists must verify AI-generated insights against established guidelines and their professional expertise.

Looking Ahead: The Future of Clinical Pharmacy

From printed textbooks to mobile apps to AI-driven tools, clinical pharmacy has come a long way. These advancements have not only changed how pharmacists work but have also improved patient care by making drug information more accessible and up to date.

Looking forward, AI will likely continue to evolve, integrating with electronic health records and real-time clinical decision support systems. As pharmacists, we must embrace these changes while maintaining our core role as medication experts who ensure patient safety and effective treatment.

The journey from 2010 to 2025 has been remarkable, and the next decade promises even more innovation (Yes! Pharmacointelligence). One thing is certain: technology will keep advancing, but the human touch in pharmacy will always be irreplaceable.

To build a winning team it's essential to build a culture of caring. To build a culture of caring you must be a leader who cares. When you care, you will inspire others to care. Find ways to extend yourself to others and serve them. Write a note. Make a call. Go out of your way to serve someone. Go beyond the expected. People know you care when you go out of your way to show them they matter. A smile, an encouraging word, an extra five minutes of time, solving a problem, listening to an employee, sacrificing for a friend, and helping a team member through a challenging time can make all the difference. Never underestimate the importance of making time to make someone feel special. Then, when you develop a reputation for caring and others expect more from you, you continue to deliver more than they expect. With each caring act you are saying, “I am here to love you and serve you,” and when this happens, you attract more love and success. Your team will love working with you. People will tell stories at parties and gatherings about you. Authors may even write books about you. Caring is the ultimate team-building strategy. People make it complicated but it's simple:

  1. Care about the work you do.
  2. Surround yourself with people who care.
  3. Show your team you care about them.
  4. Build a team that cares about one another.
  5. Together show your customers/fans/students/patients that you care about them.

When we make caring a strategy and create a culture of caring, we stand out and create success that lasts.

十月风暴

七月时,几度zero local transmission,原以为马来西亚已经成功控制新冠肺炎。

殊不知,我们经历十月风暴。沙巴州选后,第三波covid19几乎击溃沙巴的医疗,最糟时沙巴99.5%医院covid病床已满。 其他州也多处变成红区,24日更是首次破千,1228新增案例。家乡雪州更是多县沦陷,很大可能成为下一个epicenter 。

每天不断攀高的病例,每天都有covid死亡病例,让人忧心忡忡。没有州可以独善其身,在柔佛的我也接到了医院冻结假期的指令。大家都进入备战状态。

国难当头,不求减轻责任,只希望可以有更多的能力去付出自己微小的力量。

希望covid风暴后,能和老婆家人再次相聚。现在我们先努力加油吧! 

在这紧要的关头,每个人都是前线。“世上没有从天而降的英雄,只有挺身而出的凡人”,大家共勉之。

从华人农历新年到马来人开斋节

还记得JKNJ一月时 pembekuan cuti,我缩短了自己的农历新年假期,提前开工。

行动管制从三月十八日开始,现在已是第六十七天,五月的开斋节还是无法跨州回家。

如果不算video call,也已经超过几个月没有看到我老婆。

我不知道几时会回到以前的日子,也不知道几时会看到自己重要的家人和爱人。

他们说要适应New Norms。

"biar jauh di mata namun sentiasa dekat di hati"

工作,放工,休息,工作,放工,休息,周而复始。在医院工作,害怕被感染,更担心自己传染给别人。宅在屋子,每天说话最多的对象是隔着荧幕的老婆。

其实差不多每位去到医院的病人都会用到药,我们药剂师让医院正常运作也是很重要的。

我想很多同事也很想家吧,毕竟很多也都是外州人。因为服务病人的使命,加上行动管制,他们无法回乡。

往好处想? 虽然病例超过七千,但至少现在我们成功flatten the curve,这是每个马来西亚人民的功劳。希望意大利美国英国巴西的大量covid19死亡病例不会在马来西亚发生。

看到新闻全球病例超过530万。
每次想到自己无法控制的东西,心情就会起伏不定。

我们几时才能重获真正的自由?

这场抗疫战争还没结束。

武汉肺炎

2020年刚进入五月而已,却仿佛过了很久很久。

毫无疑问,中国对于武汉肺炎是有所隐瞒。今年1月3日至1月16日,武汉官方报告只有41人确诊。
这造成世界卫生组织在1月14日的推特:
 "Preliminary investigations conducted by the Chinese authorities have found no clear evidence of human-to-human transmission of the novel #coronavirus (2019-nCoV) identified in #Wuhan, #China"

现在看回去,当时的推文显得格外刺眼。

当然,我是不赞成美国停止资助世界卫生组织。毕竟我们现在与病毒第三次世界大战。至少世卫有亡羊补牢,1月30日宣布covid-19为 public health emergency of international concern,当时covid-19在中国以外的18个国家只有98个病人。

接着我们看到意大利医疗因为病人太多的崩盘,欧洲各国的沦陷。世卫也在3月11日宣告pandemic,当时全球114国家超过12万人感染。

现在全球covid-19已超过320万人感染,锁国变成一个常态,这是半年前没有人会想象的情况。

乱世中有救国英雄,也有混世魔王。非常讨厌自私自大常说谎的Trump,美国有超过100万人确诊,超过6万人丧命,他很大部分是罪魁祸首。

整个四月最好的新闻就是Dr Anthony Fauci 宣布Remdesivir 有显著的效果。

马来西亚行动管制进入第45天。
原本以为513 会有新的意义(行动管制的结束),
政府却选择提前放宽了,让大部分的行业在5月4日开工。Lives vs Livelihood,exit strategy也是很多国家在热议的。病死或是饿死。

我是非常感谢中国对于马来西亚在这肺炎的帮助。
但坦白说,我不太相信中国的官方数字,感觉上它动用了很多传媒企图改写历史,我很难忘记李文亮,一个人口13亿的大国,只有8万多确诊病例,你相信吗?

马来西亚的朋友们,大家不可以松懈。
这场硬战还没结束。希望没有第三波。


数据不单单只是数据。每个死亡数字背后都是一个故事,都是一个家庭失去了家人的悲伤和疼苦。

莫让数据飙升,
别让悲剧发生。

锁国第十一天

3月25日时,首相又把锁国延长到4月14日。
这次大家都没有慌张,因为预料之中,
这场抗疫战争没有这么快可以停息。
马来西亚锁国一个月其实是无可避免的,我们要坚强挺下去。

随着欧洲沦陷后,转眼间,美国已成为灾情最严重的国家。意大利灾情还没有看到好转的痕迹。直至3月28日下午,全世界已要接近600千的人受感染。我常想起当时不畏强权想公开疫情的李文亮医生,愿你安息。

马来西亚很幸运有Noor Hisham bin Abdullah 当卫生总监。他的领导,至少让前线安心。

真的,他说的没错。他提出Failure is not an option.他也强调 It's better to be overprepared than underprepared. Underprepared means unprepared.

马来西亚已经有超过2千个covid-19病人,26人往生,迫切希望疫情可以被控制。

锁国第一天

covid-19 导致了马来西亚被逼从18日到31日锁国。人民却出现了大量返乡或抢购潮,非常令人痛心。这个紧急时候,政客还搞政治,不让五个州领袖参与会议。18日马来西亚已有790个covid 19病人,2个病人不幸丧失性命。这些事都让我们身为医疗人员心力交瘁,我们自知我们的医疗设备非常有限,如果连有6000icu beds 的意大利都撑不住,我们是非常焦虑为何领导层没有迅速提出应对方案。

“Please stay at home as much as you can... we cannot cope if covid-19 spread widely... italy healthcare  collapsed in just 2weeks (now they got more than 30k cases)... the following 2weeks will be the most important period in the history of Malaysia, the fate of Malaysia depends on us, we are all in this together, thanks”

也只是希望
疫情结束后
能再次见到老婆
看到自己的家人

29岁

好久没有写blog了,或许找到一位可以无所不聊的女友,写blog的自言自语变得可有可无。

今天纯碎想写下进入29岁第一天自己对所发生一切的感恩。
进入29岁:
必须承认幸福感满满,有伴的感觉很好。
爱情上我每次看着凤妮,就非常感恩自己多么幸运能够和她在一起。
最后一个2字头的生日,很幸福,有女友陪伴度过。
我爱你,凤妮。




事业上,身为临床药剂师,我每天都可以做自己喜欢的事,尽自己小小的力量,帮忙医生调整病人的药物,和教导病人正确使用药物。指导PRP也是我很喜欢的事,毕竟个人力量有限,我们需要团队,更多年轻人的热血。Make patients your first concern, 当年在大学所学的一句话,一直记在心里,希望自己莫忘初衷。

2017年回顾 =)


过去几年年尾都有写回顾,2017年的年尾却忘了写,匆忙度过。

朋友问我:“What 2017 meant for you?"
当时我说:Overall it's a good year

整体来说,2017年是一个好的一年。
“学习”是我选择的年度关键词。





2017年头家里连续四个人因为Dengue入院是一个我难忘的经历。健康真的最重要。
2017年也是我第一次和妈妈家人们出国旅行,日本真的是很值得去的地方。
















2017年也去了新加坡,见到了多年不见的朋友。




接管DMTAC满一年,是个不错的体验。遇到很多不同故事的病人。曾遇过一个病人告诉我,她一个月只有RM150的补助金,蔬菜对她来说很昂贵,她几乎都是饭配咖喱过日子。也难怪她血糖控制很差。我无法想象自己如何以RM150过一个月。社会真的有太多弱势组群等着我们去帮助。























是的,没想到,我留在昔加末超过两年了。2018年,昔加末也从Majlis Daerah Segamat升格为Majlis Perbandaran Segamat。这两年多来,看了几批PRP完成了实习,之后去不同的地方服务。也有不少同事已经调去别的地方。2017年,我顺利从UF41升职到Pegawai Farmasi UF44。最重要,年尾的Jabatan Meeting提到我2018年9月将接手一个去升学的同事,成为Medical Ward的Clinical Pharmacist。这个消息让我兴奋了很久。我会继续不断自修学习! 众课本请继续赐我知识与力量,哈哈。加倍努力!















































2017年也参加了四场婚礼。每一场都洋溢着幸福。
内心会有小小的声音:希望有一天,我也会拥有属于我自己的幸福。









面对一切,除了感恩,还是感恩,希望2018年会更好。
